Yoga Liability Waiver for Children and Teens

I herby agree to the following:

My child is participating in classes or services during which she/he will receive information and instruction about yoga and wellness. I recognize that yoga requires physical exertion, which may be strenuous and cause physical injury, and I am fully aware of the risks and hazards involved. I understand that it is my responsibility to consult with a physician prior to and regarding my childs participation in any physical fitness program, including yoga. I represent and warrant that my child has no medical condition that would prevent her/his participation in physical fitness activities.

In consideration of being permitted to participate in the yoga classes, I agree to assume full responsibility for and risks, injuries, or damages, known and unknown, which my child might incur as a result of participating in the program, including injuries due to the sole negligence of Lotus River Yoga and Wellness, LLC (LRYW), or its instructors. In further consideration of being permitted to participate in yoga classes, I knowingly, voluntarily, and expressly waive any claim I may have against the instructor, the owner, or the leaseholder of the building for injuries or damages that my child may sustain as a result of participating in classes, sessions or workshops held by LRYW, its owner and its instructors.

LRYW takes the COVID-19 pandemic and your child’s safety seriously, and for in person yoga, social distancing, mat cleaning, and other measures may be required before class. No instructor that has been diagnosed with or exposed to Covid-19 in the last 14 days shall be allowed to teach until 2 negative tests have been received. If attending in person classes or sessions, I understand the risks of Covid-19. I represent and warrant that within the last 14 days my child has not had any symptoms of Covid-19, has not been exposed to someone known to have been diagnosed with Covid-19, and has not tested positive for Covid-19. I agree to immediately notify LRYW if my child develops symptoms, is exposed to, or diagnosed with Covid-19 so that appropriate quarantine steps can be taken by others within the class. LRYW, its owner and instructors will not reveal the identity of any child exposed to or diagnosed with Covid-19.

I waive and release all claims, damages and liability that may arise from my child’s Covid-19 infection that in any way arises from or could possibly relate to their participation in any activities with LRYW or its owner and instructors.

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
